Accident Investigation
Accident investigation is the process of analyzing and determining the causes and contributing factors of an accident or incident that occurred during space and astronautical engineering activities. The investigation aims to identify the root cause of the accident, evaluate the effectiveness of existing safety measures, and develop recommendations to prevent similar accidents from happening in the future. Accident investigation involves collecting and analyzing data, interviewing witnesses and involved personnel, reviewing procedures and policies, and examining physical evidence. The investigation may also involve simulations and experiments to recreate the accident scenario and test hypotheses. The ultimate goal of accident investigation is to improve safety and prevent future accidents by identifying and addressing the underlying causes and contributing factors.
